How to get a Library Card

Getting a library card is quick and free! 

For your convenience you can print the form from the link down below, complete it and bring it to the library along with valid photo ID and current address verification.  Please print legibly.

Forms can also be picked up at the circulation desk of the library.

Any of the following are acceptable forms of identification: drivers license, utility bill,  mortgage papers, or state identification card.

If you lost your card you can request a new one for a $1.00 fee.

Children may obtain a library card beginning at the age of five. Any patron under the age of sixteen must have a parental guardian to sign the registration forms.

Library cards can be used in any public library in Adams, Columbia, Dane, Green, Portage, Sauk, or Wood counties.
